Indications/Uses
Prophylaxis & treatment of venous thrombosis & pulmonary embolism. Treatment of MI & arterial embolism. Prevention of clotting in arterial & heart surgery; of cerebral thrombosis. Anticoagulant in blood transfusions, extra-corporal circulation, dialysis procedures, & for lab purposes.
Dosage/Direction for Use
Haemodialysis 7,500-12,500 IU (w/o preservative) per dialysis. IV 5,000-10,000 IU every 4 hr either by bolus inj or continuous infusion in NaCl or dextrose inj. SC 10,000 IU every 8 hr after an initial IV bolus inj of 5,000 IU. Low-dose heparin prophylaxis 5,000 IU in 0.2 mL SC, given 2-6 hr pre-op & every 8-12 hr post-op for 10-14 days or until the patient is mobile, whichever is longer. MI 5,000 IU SC every 12 hr beginning during the 12 hr following the 1st sign of MI. Open heart surgery For operations <2 hr: 120 IU/kg/hr. For operations of longer duration: Administer 1½ times the dose. For each 450 mL of blood used, 2,000 IU are needed. Treatment periods vary from 10-14 days in peri-op prophylaxis to as much as 6 wk in the treatment of established thrombosis.
Contraindications
Hypersensitivity. When suitable blood coagulation tests eg, whole-blood clotting time, partial thromboplastin time cannot be performed at the required intervals. During any uncontrolled active bleeding state. Heparin w/o preservatives should be used in premature infants.
Special Precautions
IM route is not recommended due to the high incidence of haematoma. Regulate dose by frequent blood coagulation tests. Promptly discontinue if coagulation test is unduly prolonged or if haemorrhage occurs. Increased danger of haemorrhage in subacute bacterial endocarditis, arterial sclerosis, increased capillary permeability, during & immediately following spinal tap/anaesth or major surgery (especially involving the brain, spinal cord or eye); conditions associated w/ increased bleeding tendencies eg, haemophilia, some purpuras, & thrombocytopenia; inaccessible ulcerative lesions & continuous tube drainage of the stomach or small intestine. May prolong the 1-stage prothrombin time. Caution in patients about to undergo surgery; w/ history of allergy; w/ renal or hepatic impairment. Long-term (≥6 mth) application can cause severe osteoporosis in pregnant mothers. Completely stop heparin therapy 6 hr before delivery. Elderly.
Adverse Reactions
Transient alopecia, diarrhoea; thrombocytopenia & osteoporosis w/ spontaneous fractures. Occasionally, febrile or allergic reactions.
Drug Interactions
Induced bleeding w/ drugs that interfere w/ platelet-aggregation reactions eg, ASA, dextran, phenylbutazone, ibuprofen, indomethacin, dipyridamole, hydroxychloroquine. Partially counteracted anticoagulant action w/ digitalis, tetracycline, nicotine, or antihistamines. Increased drug resistance is frequently encountered in thrombosis, thrombophlebitis, infections w/ thrombosing tendencies, MI, cancer, & postsurgical patients.
